Questionnaire

At the time of the DEXA assessment, the patient is required to fill out a clinical questionnaire relevant to osteoporosis. This questionnaire helps to:

• determine any risk factors associated with osteoporosis
• determine any clinical history which may affect the results
• determine Medicare eligibility of the patient
• prepare the report that is sent to the referring doctor
